Medicare, DVA and Private Health

As exercise physiologists, all ExCo staff are professional allied health providers who are recognised by Medicare, DVA and most private health funds. Clients referred to ExCo for a health condition or injury may be eligible to claim subsidised consultations and/or exercise sessions.

Conditions covered include:

  • Osteoporosis/osteopaenia
  • Injury rehabilitation
  • Osteoarthritis
  • Cancer
  • Chronic pain
  • Cardiovascular disease
  • Type II diabetes
  • Hypertension (high blood pressure)
  • Neurological conditions (eg MS, peripheral neuropathy, Parkinsons Disease)
  • Respiratory conditions (eg asthma, COPD, emphysema)
  • Endocrine/hormonal dysfunction (eg PCOS, thyroid conditions)
  • Mental health (eg depression, anxiety)

In order to be eligible for your subsidy, the following details are required:

  1. A GP management plan/team care arrangement
  2. Supporting referral form for Individual Allied Health Services under Medicare that has the correct details, is signed and has the allocated number of Exercise Physiology (item 10953) services indicated. Up to five services may be claimed per calendar year, and a rebate of $56 is applied per consultation.
  3. Your Medicare card number.

* Note for those with Type II Diabetes, there is a specific supporting referral form for Group Allied Health Service under Medicare. This may be used for claiming a subsidy for an initial assessment, and up to either group classes/services per calendar year.   1. Referral from your GP, medical specialist, hospital discharge planner or other healthcare professional as part of a hospital discharge specifying the particular health condition/conditions to be treated or managed.
  2. D904 referral form from your GP, specifying the particular health condition/conditions to be treated or managed.

For most gold card and white card holders, up to 12 consultations can be claimed per referral. Following this, your suitability for a new plan will be made in consideration of your progress, and in consultation with your referring practitioner. For more information, visit this LINK

This includes any referral from a GP, medical specialist or other treating health professional. The goal of the exercise physiology plan is to develop a program to follow and learn to self-manage your condition and optimise your health outcomes.
